
Program Coordination
Each person who receives services at DakotAbilities has a Program Coordinator. This person is responsible for
coordinating all supports and services. The Program Coordinator acts as a primary advocate for the person and
ensures that the person's needs are identified and addressed.
DakotAbilities utilizes an interdisciplinary team approach in providing services. Team members include the
person and his/her family members or guardians. Other members may include an advocate, staff from DakotAbilities
and consultants as appropriate. The Program Coordinator is also a member of the team.
The Program Coordinator is responsible for the facilitation of an Individual Service Plan (ISP) for each
individual. During the ISP meeting, the team identifies strengths, needs and goals and develops an
individualized plan. The Program Coordinator is responsible for monitoring the plan for progress and
assuring that the plan is being implemented as intended.
The Program Coordinator also serves as the primary contact for family members, guardians and advocates.
Additional responsibilities include the facilitation of all team meetings, assisting the person with
community integration, funding eligibility, benefits, management of finances, assuring health services
are provided and locating and maintaining affordable housing as appropriate.
